What makes lips look overfilled, and what good proportions actually look like?
The classic overfilled appearance, sometimes called duck lips, happens when volume is distributed incorrectly. It tends to result from placing too much product in the body of the lip, particularly the upper lip, without respecting the natural proportions of the face. The upper lip should ideally sit slightly smaller than the lower lip in terms of projection. When that ratio is reversed or exaggerated, the result looks artificial regardless of how carefully the product was placed.
Overfilled lips can also lose their natural contour. The philtrum columns, the two ridges that run from the base of the nose to the lip, are an important structural detail. When filler disrupts these or obscures the cupid’s bow, lips start to look like a single, inflated shape rather than a defined, nuanced feature. Definition and shape are what make lips look beautiful. Volume alone does not.
The other common cause of an unnatural result is simply using too much product in a single session. Part of my job is to guide that conversation honestly, which sometimes means recommending less than a patient initially had in mind.
Not all lip fillers are the same, and the difference shows
Not all hyaluronic acid fillers behave the same way. Some are designed for structure and projection; others are softer and better suited to subtle volume enhancement. Using the wrong product for the concern, or for a particular patient’s anatomy, is one of the most common reasons lip filler looks heavy or unnatural.
For patients seeking a genuinely natural outcome, I favour softer, more pliable formulations that integrate well with lip tissue and move naturally with facial expression. Lips are dynamic. They move constantly when you talk, smile, eat and laugh. A product that behaves naturally during movement will look natural far more consistently than one that has been chosen for a static photograph.
For patients who want a little more definition or projection rather than overall volume, there are products better suited to work along the vermillion border or in the philtrum columns. Knowing which product to use in which zone, and in what quantity, is where clinical experience makes a genuine difference.
Where filler is placed matters more than how much is used
Placement is arguably the most important factor in achieving a natural lip filler result. A product placed too superficially can create lumps or a bumpy texture under the skin. A product placed in the wrong anatomical zone creates distortion. Correct placement into the appropriate tissue planes gives even, smooth volume that looks as though it belongs there.
I use a combination of needles and cannulas depending on the area being treated and the result we are aiming for. A cannula, a blunt-tipped flexible tube, allows product to be deposited through a single-entry point, which reduces bruising and swelling significantly and makes it easier to distribute filler evenly across a larger area. It is not always the right tool for every zone of the lip, but for volume enhancement in the body of the lip, it tends to produce a gentler, more even result.
The injection points, depth and the volume placed in each location are all decisions made in the moment, adjusted to what I see in front of me rather than a fixed protocol. All patients’ lips are different in shape, tissue density and starting volume, which is why a thorough consultation always comes before any treatment.
Why I almost always start with less than patients expect
The single most important principle in achieving natural lip filler results is this: it is far better to add more at a follow-up appointment than to place too much and need to dissolve it. Hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase, but dissolving is not a neutral process. It can cause temporary swelling and unevenness, and it is far more disruptive than booking a small top-up a few weeks later.
For patients having lip filler for the first time, I almost always recommend starting with a conservative amount, typically 0.5ml to 1ml, depending on the patient’s existing anatomy, and reviewing the result at four to six weeks. By that point, the filler has fully settled, any initial swelling has resolved, and both the patient and I can see exactly what we are working with. If more volume is needed, we add it then.
This approach takes longer, but it consistently produces better results because every decision is made from an accurate picture rather than an estimate.
The consultation is where natural results begin
No amount of good technique overcomes a mismatch between what a patient wants and what their anatomy can realistically support. This is why I spend time in every consultation understanding not just what someone wants their lips to look like, but what result would genuinely suit them.
I look at the overall balance of the face, the proportions of the lips at rest and in movement, the quality of the surrounding skin and the patient’s natural structure. Sometimes, the most impactful change I can make for a patient who wants fuller lips is to work on definition and projection rather than volume, creating the appearance of size through shape rather than bulk.
I also talk honestly about what filler cannot achieve. It is not the right treatment for every lip concern, and there are patients who would benefit more from a different approach. That conversation always happens in the consultation, before anything is injected.
